Report an Incident
The Spore Foundation is committed to creating a safe, ethical, and supportive environment for all members and ministers. While we strive to live in right relationship with one another, we also recognize that misunderstandings, conflicts, or violations of our Code of Ethics can occur.
This is a confidential process for reporting any incident that raises concern. Reports may be submitted by members, ministers, or guest participants, and each will be reviewed with care and integrity.
Our goal is to address issues promptly, fairly, and in alignment with our values of reverence, compassion, and accountability.
Instructions for Reporting:
Review the Code of Ethics and the Internal Incident Response Policy to help determine whether the incident falls within its scope.
Complete the Incident Report Form with as much detail as possible. You may choose to identify yourself or remain anonymous. (Please note that anonymous reports may limit our ability to fully investigate the incident.)
Submit the form.
Once submitted, the report will be reviewed by the Steward Committee, and appropriate follow-up steps will be taken.
